Alertness Log

Many students have a hard time concentrating or staying alert in school all day. You can do a self-check of your alertness during the day by filling out an Alertness Log.

DIRECTIONS:

  1. To start, fill in the chart with the day’s classes or activities.
  2. After each class or activity, mark the box that best describes you.
  • If you were alert or "On" for the whole time… check On.
  • If you were "On" for some of the time, but were tuned out or distracted at other times… check On & Off.
  • If you couldn’t concentrate at all…check Off.

At what times during the day were you most alert ("ON")? Why?
At what times were you sometimes alert and sometimes distracted ("ON" & "OFF")? Why?
At what times were you not alert at all ("OFF")? Why?
